How about a modern retelling of Romeo and Juliet, but with a few subtle tweaks?

Juliet's parents are the heads of a local gang. One day she meets Romeo, and falls in love. Only problem is that Romeo's parents are the heads of a rival gang. They profess their love for each other. Romeo's cool friend Mercutio gets killed when Juliet's brother Thibault (yes, THAT thibault!) shoots him. Romeo kills thibault in revenge. People start moping around. Romeo gets banished by the Artist Formerly Known as Prince. Juliet is forced to marry a guy named Paris (who, based off his name, is probably a wuss). Romeo, heartbroken, consults the good friar to find a way to marry Juliet. Then David Hasselhoff kills everyone with his kick-ass laser eyes, and flies off with Stella Star, living happily ever after.
There is no mafia. It's a figment of your imagination. Here, try this cannoli. A box of them fell out of a refrigerated delivery truck. Today's your lucky day.

It's a very cool genre. A very common thing in them is a femme fatale. So it's an attractive and "wild" woman that seduces the hero and gets him to do criminal things - and in the end it turns out the woman was just using him as a tool.
There are many real life mafia stories worth telling ,like them and the Pope and many other Italian powerful are all in a secret cabal to rule the world.
Even the current Southern Italy mafia story about them working with the Nigerian mob to smuggle human traffic through the desolate Southern coast the blacks over running whole cites that where abandoned, then the underdog law enforcement finally ending that family.
Remember there weren't much mafia before the kingdom of the two Sicilies was dissolved. You only have these kind of crime families in Republics.
